Dear Friend:

Shalom’s primary care clinics:

  • provided over 16,000 patient visits in 2007
  • primary emphasis is on high quality health care for patients of every age
  • collaborate with health care professionals to provide accessible, affordable health care for the uninsured
  • grants and donations provide the majority of Shalom's support patients pay on a sliding fee scale; however no patient is turned away for inability to pay

Shalom Health Care Center, Inc.  provides a wide range of primary health care services including:

  • acute care
  • health maintenance
  • screening
  • chronic disease management
  • health education
  • diet and weight management
  • smoking cessation

We would appreciate a gift from you to:

  • help obtain additional space for the clinics
  • contribute to operating expenses to fulfill our mission of providing affordable and accessible healthcare

Your gift will help provide an adequate clinic and operating funds to care for a child, an elderly person, or a hard working uninsured adult.  Here are some examples of how you could help in the operation of the clinics:

  • $ 25 helps fund a child’s immunization visit
  • $ 50 helps treat a child with asthma
  • $ 100 helps detect cervical and breast cancer
  • $ 500 helps fund the clinic’s lab tests for a day
  • $ 1,000 helps provide treatment of a patient’s diabetes for six months
  • $ 2,000 helps sponsor a day of healthcare in one of Shalom’s primary care clinics
  • $ 10,000 helps fund one of the primary care clinics for a week.
  • Other: $______________
